3. Enter the cashier code from your receipt.
4. Enter the 16-digit code, found on the bottom of receipt.
5. Click on Next button to continue the survey.
6. You will be asked to rate some situations. Rate them according to your like and dislike.
7. Answer a series of questions regarding your most recent experience at the store. Please provide honest and relatable answers to the questions.
8. At the end, submit the survey.
